Registration Checklist

 

To register as a new student, you will need to call Mrs. Talsma at 248-823-2715 to set up an appointment.

Appointments are required to register -- walk in registrations are not allowed.

The following items must be brought with you at the time of registration

 

  • Proof of Residency (one of the following required):
    • Closing Statement
    • Warranty Deed
    • Mortgage Statement
    • Current property tax statement
    • Lease Agreement
    • Notarized Residential Affidavit
  • Two of the following required:
    • Bank statement
    • Credit card statement
    • Utility bills (phone, electric, or gas)
    • Insurance document (automobile, homeowner’s, health, etc.)
    • Other
  • Certified Birth Certificate
  • Record of Immunizations
  • Special Education Placement Sheet (If student has been in Special Education Classes, contact Ms. Linda Hannon at Special Education at 248-823-5096 before registration appointment.)
  • Social Security Number of student (voluntary)
  • Unofficial Transcript of student (IF FOREIGN TRANSCRIPT - MUST BE TRANSLATED)
  • Grades at the time of withdrawal from former school
  • MEAP scores (if entering from a Michigan school)
  • Schedule from former school (If registering during school year)
  • GUARDIANSHIP PAPERS OR POWER OF ATTORNEY (if not residing with parent.) If using Power of Attorney, must also complete Parental Affidavit form and Relative Affidavit form, indicating that placement is not for educational purposes, but that parents are unable to provide a suitable home for child.
  • Emergency card must be completed and returned before student receives schedule (during the school year)
