( The CA-6 is a pink form used for minors who are under 16. The CA-7 is a yellow form and is used for minors who are 16 and 17 years of age.
2. The student completes Section I. 3. The student takes the form to the prospective Employer who completes Section II. 4. The student takes the form to the Counseling Office Support Staff.
Section III of the form will be completed by Counseling Secretary (if the work permit complies with state and federal laws and regulations). 5. Student returns the completed original form to the employer.
