1. Students may pick up a blank work permit in the Counseling Office ( The CA-6 is a pink form used for minors who are under 16.
The CA-7 is a yellow form and is used for minors who are 16 and 17 years of age.) 2. The student completes Section A.
3. The student takes the form to the prospective Employer who completes Section B. 4. The student takes the form to the Counseling Office so we as a school can fill in Section C.

For students interested in health care volunteer experience, try calling local nursing homes to get experience working with patients. Nursing homes tend to be more flexible than hospitals with age
requirements, and hours that you work. Sometimes local churchs ask for volunteers to work at free medical clincs in Pontiac or Detroit. Check with your church
